Accessing and completing forms

Once an appointment is confirmed, any form or report requirements that have been linked to the competition type and role will start to appear against the game. Forms may be required before or after the game, as set by your organisation Administrator.

There are two main types of forms that will automatically appear for completion:
  1. Refbook forms: these forms have been built using Refbook's inhouse form module
  2. External forms: these are links to external online forms that have been built by your organisation in a third-party form builder (eg: Cognito Forms, JotForm, Google Form)
Accessing and completing forms
  1. Go to My Appointments>My Current to access your games.
    1. If the game date/time is in the past ('post-game' form), change the Status filter to 'All' or 'Finalised' in order to see the historic games.
  2. Click the Game Portal icon in the Actions column 
  3. Click the Forms tab.



  4. Click My Forms. This tab displays all forms that you are required to complete according to your Position.



    1. Note: 'Shared Forms' refers to forms that have been completed by other members of the officiating crew on your game, and configured to be shared with your Position (once they have completed).
  5. To start completing the form, click the To Do icon 
  6. Fill in the questions as required
  7. Click Submit. The form will be marked as Completed 
Note: forms can also be saved in Draft format to be recovered and completed at a later time.
