'Shared Forms' are forms that have been completed by other members of the officiating crew and configured to be shared with your position once completed. A common example of a shared form application is a performance assessment report completed by a referee coach/instructor.
Shared forms are made and completed within the platform and do not have connection with any external form builder.
Viewing completed Shared forms
- Head to 'My Appointments' to access your confirmed games.
- Change the Status filter to 'All' or 'Finalised' in order to see the completed games.
- Click the 'Game Portal' icon in the Actions column

- Click the 'Forms' tab.

- Click 'Shared Forms'. This tab displays all forms that have been completed and shared with your position in the officiating crew.

- Click the 'View' icon to access.

- Click 'Back' to return to the Forms homepage
