Viewing Shared forms

'Shared Forms' are forms that have been completed by other members of the officiating crew and configured to be shared with your position once completed. A common example of a shared form application is a performance assessment report completed by a referee coach/instructor.

Shared forms are made and completed within the platform and do not have connection with any external form builder.

Viewing completed Shared forms
  1. Head to 'My Appointments' to access your confirmed games.
  2. Change the Status filter to 'All' or 'Finalised' in order to see the completed games.
  3. Click the 'Game Portal' icon in the Actions column 
  4. Click the 'Forms' tab.



  5. Click 'Shared Forms'. This tab displays all forms that have been completed and shared with your position in the officiating crew.


  6. Click the 'View' icon to access. 
  7. Click 'Back' to return to the Forms homepage
